The wellbeing services county of Central Uusimaa (Keusote) provides social and healthcare services to approximately 200,000 residents across the municipalities of Hyvinkää, Järvenpää, Mäntsälä, Nurmijärvi, Pornainen, and Tuusula.

Before implementing the Uoma system, patient transfers were primarily coordinated by phone and fax. This slow process strained the healthcare coordinators, made information flow sluggish, and led to frequent errors. Each transfer could take up to an hour, and the coordinators struggled to maintain an overview of patient flow across the region.

Our solution

In 2019, Keusote introduced the Uoma system to standardize and digitize patient transfer processes. Initially launched as a pilot, Uoma quickly expanded to cover services like Hyvinkää’s emergency department, the rehabilitation center, home hospital, home care, and client guidance. Today, Uoma is widely used across Keusote's units, facilitating smooth integration of patient transfers between Keusote and specialized healthcare services. Coordinators now manage patient flow across hospital services at a regional level and can adjust transfer directions as needed, which has been particularly beneficial for the success of home hospital services.

“Uoma is extremely user-friendly and straightforward. Where one transfer used to take an hour, it now takes less than a minute,” says Susanna Laakso, healthcare coordinator at Keusote.

Uoma has significantly improved Keusote’s hospital patient transfer management. Transfers are now centralized, allowing coordinators to flexibly monitor the availability of patient beds across the region. Transfers between hospital departments and home hospital services are streamlined to suit each situation, enhancing the flexibility and continuity of patient care. Currently, Keusote’s hospital services handle around 800 patient transfers each month – a volume that would be impossible to manage without Uoma.

Additionally, Uoma ensures accuracy and traceability, which enhances patient safety and reduces the risk of errors. The system’s design also allows for future integration of social services, further expanding Uoma's role. “Patients don’t end up in the wrong place at the wrong time – Uoma helps us keep all transfers under control,” Laakso concludes.
