Case Wellbeing services county of Southwest Finland

Starting point

Managing patient transfers and care coordination required significant effort, and communication between units was sometimes challenging. Determining the availability of patient beds was largely based on phone calls, which burdened staff and took time away from patient care.

Our solution

The Wellbeing services county of Southwest Finland (Varha) implemented the Uoma system to optimize patient logistics and care coordination. With Uoma, patient transfer management has become more transparent, and care coordination can now be carried out more efficiently and in a more patient-centered way.

Benefits

Uoma has significantly reduced phone traffic, freeing up staff time for actual patient care. Patient transfers are now faster and safer, as all necessary information is readily available in the system.

Seamless and efficient cooperation

Uoma Project Coordinator Anna Knifsund says that working with Axel Health has been smooth and efficient. "Working with them is easy, and asking for help feels effortless – you never have to worry about whether you should or can reach out. Support is always available, and responses are quick."

Starting point

The Wellbeing services county of Southwest Finland (Varha) is Finland’s second-largest wellbeing services county in terms of staff and it provides healthcare and social services for approximately 500,000 residents.

In Varha's different units, managing patient beds and care coordination relied heavily on phone calls, increasing staff workload and causing delays in patient transfers. Communication between different units was not always seamless, and checking the availability of beds was time-consuming. Finding an available bed required calling multiple units, which took up valuable time and resources.

"The best thing about Uoma is its ease of use and clear operational logic.
Additionally, communication via chat is extremely easy and smooth.
I can wholeheartedly recommend Uoma."

Our solution

In 2023, the Uoma system was gradually implemented across various units, including emergency care, specialized and general medical wards, geriatric wards, home hospitals, palliative care center, and social and healthcare center outpatient clinics.

With Uoma, patient bed availability is now visible in real time, and all necessary information is easily accessible from a single location. Patient transfers can now be managed without separate phone calls, streamlining operations and enhancing patient safety.

"Uoma also provides a clearer overall picture, as it provides a precise view of where and how many available beds there are," Knifsund explains.

Benefits

With Uoma, patient transfers are now smoother, allowing staff to focus more on patient care. The reduction in phone traffic has been a significant advantage, and the system's clear operational logic has received positive feedback from users.

Reporting has also improved, and patient bed availability is now easier to monitor in real-time.

"Additionally, the Uoma support team has been extremely responsive whenever any issues have arisen. According to the Uoma user survey, Varha’s users find the system to be an easy-to-use tool for patient transfers," Knifsund adds.

Varha is also currently implementing an integration between Uoma and the patient information system, which will further streamline operations and reduce the need for manual data entry.
